Who We Are
The Family Healthcare Foundation seeks to be the leader in providing community-based Healthcare Navigator services in Hillsborough, Pasco, Pinellas, and Polk Counties.
What We Do
The Family Healthcare Foundation connects individuals and families in Hillsborough, Pasco, Pinellas, and Polk Counties to healthcare options that meet their unique healthcare needs.
How We Do It
The Family Healthcare Foundation provides Healthcare Navigator services absolutely free of charge, to everyone, in multiple languages. We also convene community partners to support collaborative efforts to promote access to affordable healthcare.
Throughout the 4-county area, Healthcare Navigators are available to help you complete an application and see the available health plan options through the Health Insurance Marketplace. You also can find out if you qualify for free or low-cost coverage available through Medicaid, the Children’s Health Insurance Program (CHIP), or programs specific to certain counties.

Healthcare Navigators represent the communities we seek to serve.
We strongly believe that our learning never ends. Healthcare Navigators engage in continuing education throughout the year, and complete yearly certification to ensure licensure compliance.
We are committed to free, objective, and confidential assistance to any person, regardless of need. If we do not know the answer to a question, we will provide a warm connection to an agency that will.
